Statewide Health Care Facility Transformation Program IV & V

The New York State Department of Health and the Dormitory Authority of the State of New York have opened applications for the Statewide Health Care Facility Transformation Program (SHFTP) IV and V. Applications for projects focused on supporting technology, cybersecurity, and telehealth that facilitate healthcare transformation activities can be submitted from now until the submission deadline of March 13, 2024.

The RFA’s funding is made up of up to $150 million available via the SHFTP IV and up to $500 million available via the SHFTP V. This funding will be made available through grants to cover eligible capital, non-capital and/or working capital expenses proposed in the approved applications.

Interested applicants should propose projects that align with, and advance, the Department’s goals of:

• Improving access to health-related information for providers;
• Supporting data sharing across the healthcare continuum;
• Driving the adoption of standards in technology, data formats and interoperability;
• Improving the Department’s access to health-related data for supporting authorized work; and/or
• Supporting telehealth capabilities and access.

Who’s eligible to apply?

Applicants must be one of the following types of eligible organizations, legally existing, and capable of signing a binding Master Grant Contract with the Department of Health:

Prequalifications

Applicants must register to access the New York State Grants Gateway or Statewide Financial System to submit proposals via the Department’s online portal.

Project categories

To receive funding, eligible applicants must submit detailed project proposal(s) focused on one of the four (4) categories below:

1. Electronic health records

Project examples include consolidating infrastructure, implementing clinical decision support, and/or implementing systems to improve revenue cycle management, organization efficiency and quality of care. This may also include improvements in IT infrastructure and user experience that can improve employee satisfaction and patient experience.

2. Cybersecurity

Project examples include improvements to the organization’s cybersecurity posture, implementation of the NIST cybersecurity framework, HIPAA requirements, and meeting HITRUST certification requirements. This may also include improvements to technical infrastructure that support the implementation of security controls/policies/procedures to meet NIST requirements and improvements to the confidentiality of data.

3. Health management tools

Project examples include implementing applications that aggregate clinical and administrative data to support the provision of care, improve patient outcomes, and/or support provider participation in value-based contracts. Tools may also support understanding/controlling the total cost of care and gaps in care to support improvements in quality, patient engagement, and health outcomes.

4. Telehealth

Project examples include implementing and improving telehealth applications that enhance access to care or service provision. Projects of particular interest improve access to care in regions/areas with limited providers/specialties (ex, pediatric behavioral health).

Application requirements

Applicants must demonstrate how the requested grant funds will strengthen and protect access to healthcare services in communities by explaining the proposed project’s impact on:

1. Maintaining or improving the long-term financial sustainability of the applicant.
2. Improving the quality of care, patient outcomes and patient experience.
3. Integration, preservation, or expansion of essential healthcare services.
4. Addressing identified community need(s) and the extent to which community engagement has helped. shape the proposed project.
5. Advancing health equity.
6. Medicaid or Medicare enrollees and uninsured individuals.
7. The extent to which the applicant has limited access to alternative financing.
8. Extent to which the proposed activities and technology further advance access and capability for electronic health records, cybersecurity, health management tools or telehealth.
