Statewide Healthcare Facility Transformation Program V

New York’s Statewide Healthcare Facility Transformation Program offers capital funding grants to support eligible projects that align with NY State’s goals of transforming and strengthening its health system.

The State has earmarked approximately $1 billion in funding to establish Statewide V, which will focus on funding projects to deliver better outcomes, expand access to care, improve financial stability and enhance IT infrastructure and cybersecurity. Eligible not-for-profit and for-profit organizations can apply for grant funding to support projects proposed through an upcoming Request for Applications (RFA) process*.

  • Home care providers licensed under Article 36
  • Hospices licensed under Article 40
  • Adult care facilities licensed under Article 7
  • Independent practice associations or organizations
  • Residential facilities or day program facilities licensed under Article 16
  • Children’s residential treatment facilities licensed under Article 31
  • Behavioral health facilities licensed under Article 31 or Article 32
  • Assisted living programs
  • Community based programs
  • Primary care providers
  • Hospitals
  • Diagnostic and treatment centers or ambulatory surgery centers licensed under Article 28
  • Residential healthcare facilities
  • Clinics
  • Midwifery birth centers

  • Capital projects
  • Debt retirement
  • Working capital or other non-capital projects to build innovative, patient-centered models of care, increase access to care, to improve the quality of care and to ensure financial sustainability of healthcare providers, and develop capacity in underserved areas of the state.
